The data on the NYS Provider & Health Plan Look-Up is constantly being updated to give New Yorkers the information they need to make the best healthcare decisions for themselves and their families.


 At least every three months, health plans update information about their provider networks for the programs below:


Options offered through NY State of Health

  • Qualified Health Plans (QHP)
  • Essential Plans (EP)
  • Child Health Plus (CHP)
  • Medicaid Managed Care (MMC) Plans, including HIV Special Needs Plans (SNP)

Other Medicaid Plans

  • Managed Long Term Care (MLTC) Plans
  • Health and Recovery Plans (HARP)
  • Fully Integrated Duals Advantage With Intellectual And Developmental Disabilities (FIDA IDD) Plans

Commercial Health Plans that are not part of NY State of Health

  • Commercial Medical
  • Commercial Dental
  • Commercial Vision
  • Managed Care Organizations (MCO)
  • The Empire Plan

 Health plans are expected to update data about the providers in their networks within 15 days of any changes.


 Health plans may choose to contract with providers only at specific locations and for specific services. Depending on the health plan used in a search, the way the results display may appear differently from search to search. Some physicians only participate with certain products offered by a health plan.


 Before uploading data to this website, it is verified against multiple sources.
